Certificate of Insurance Request

Reimbursement Request

To submit a Reimbursement Request electronically, send an email to info@rugbyoregon.com, and include the following information:

• Your Name (must be Head Coach for team funds reimbursement)
• Team Name
• Street Address
• City, State, ZIP Code
• Date of Expense/Receipt
• Expense Type (Equipment, Apparel, Socials, Misc.)
• Brief Description of Expense
• Receipt Image(s) or File(s)
• Expense Total

Preseason Parent Meeting

Start the season off on the right foot with a parent meeting. Share with them what they can expect over the course of the season, a schedule, and be available to answer questions.
